School-Based Physical Therapist - Hayden, CO

Job Description

Degree in Physical Therapy, active Physical Therapist license and 1+ year of Physical Therapy experience required. Applicants who do not meet these qualifications will not be considered.

This position offers an opportunity for a Physical Therapist specializing in school-based services to support student health and development during the 2026/2027 school year. The role is based in Hayden, CO, and is a contract assignment within a school setting.

Key Responsibilities:

• Provide physical therapy evaluation, treatment, and consultation services to students.
• Collaborate with educators and other professionals to develop and implement individualized education programs (IEPs).
• Monitor student progress and maintain accurate documentation in accordance with school policies.
• Participate in meetings and communicate effectively with students, families, and school staff to support student outcomes.

Qualifications and Experience:

• Licensed Physical Therapist (PT) with experience in school-based therapy.
• Knowledge of educational regulations and school therapy practices.
• Ability to work collaboratively within a multidisciplinary team.
• Strong organizational and communication skills.

Contract Details:

• School-based contract position for the 2026/2027 academic year.
