Job Summary
The MDS Nurse is responsible for the coordination, completion, and accuracy of the Minimum Data Set (MDS) assessments in compliance with federal and state regulations. This role plays a critical part in ensuring appropriate reimbursement, quality resident care, and regulatory compliance. The MDS Nurse collaborates with the interdisciplinary team to assess resident needs, develop individualized care plans, and ensure documentation reflects the clinical status of each resident. This position may also provide direct resident care as needed and participate in the facility’s on-call rotation. Coordinate and complete MDS assessments (admission, quarterly, annual, significant change, and discharge) in a timely and accurate manner
Ensure compliance with CMS, state, and facility guidelines related to MDS and care planning
Initiate, develop, and update individualized care plans based on comprehensive assessments
Collaborate with interdisciplinary team members (nursing, therapy, dietary, social services, and providers) to gather and validate resident data
Accurately code MDS to reflect residents’ clinical conditions, treatments, and services provided
Monitor Case Mix Index (CMI) and support optimization of reimbursement through proper documentation and coding
Participate in care plan meetings with residents and families as appropriate
Review clinical documentation to ensure it supports MDS coding and regulatory compliance
Identify and communicate changes in resident condition to the appropriate team members
Conduct audits and participate in quality assurance and performance improvement (QAPI) activities
Stay current with changes in MDS regulations, RAI Manual updates, and reimbursement guidelines
Provide education and guidance to nursing staff regarding documentation and MDS processes
Provide direct bedside nursing care as needed to support staffing needs and ensure continuity of resident care
Additional Duties & Scheduling Expectations:
Participate in the facility’s on-call rotation, approximately every 8th weekend, to support clinical operations and address urgent needs
Maintain readiness to respond to staffing or clinical issues during on-call periods
Demonstrate flexibility in assisting with floor nursing responsibilities when required
Qualifications:
Current Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in good standing
Previous experience in long-term care or skilled nursing preferred
Experience with MDS/RAI process strongly preferred
Strong assessment, critical thinking, and analytical skills
Excellent attention to detail and organizational abilities
Effective communication and interpersonal skills
Ability to work independently and collaboratively within a team environment
Qualifications
Graduate from a National League of Nursing (NLN) or American Association of Colleges of Nursing (AACN) accredited nursing program is preferred. Bachelor’s Degree in nursing preferred.
Minimum of one-year experience working as licensed nurse with the senior population preferred. Specific services/positions may have additional education, training or experience requirements. Resident Assessment Coordinator-Certified (RAC-CT), management or supervisory experience preferred.
Currently holds an unencumbered RN license with the State Board of Nursing where the practice of nursing is occurring and/or possess multistate licensure if in a Nurse Licensure Compact (NLC) state. Obtains and subsequently maintains required certifications and completes required continuous education units (CEU) and competency training. Affiliation with an outside professional nursing organization is preferred.
