Job Details

This role focuses on after-hours hospice triage, supporting patients and families during evening and overnight shifts. You will serve as a key clinical resource—providing guidance, assessing patient needs, and coordinating care both remotely and in the field when necessary.

Schedule Options:

5:00 PM – 1:00 AM
1:00 AM – 9:00 AM

Responsibilities:

Provide after-hours triage support via phone
Assess patient conditions and determine appropriate interventions
Coordinate care with interdisciplinary teams
Perform occasional in-person visits as needed
Document all interactions accurately in EMR
Deliver compassionate communication to patients and families
