Memory Care Coordinator – Leadership Role in Dementia Care
We are seeking an experienced and driven Memory Care Coordinator to lead daily clinical and operational functions within our Memory Care neighborhood at a large senior living community in Lakewood. This is a hands-on leadership role ideal for someone with strong dementia care experience, team management skills, and a passion for creating meaningful resident experiences.

Position Overview
The Memory Care Coordinator is responsible for the overall leadership, clinical oversight, and daily operations of the Memory Care neighborhood. This role works closely with the Health & Wellness Director and Executive Director to ensure residents receive high-quality, person-centered dementia care while maintaining regulatory compliance and operational excellence.
This is not an entry-level role—we are seeking someone who can confidently lead a team, manage care delivery, and respond to changing resident needs.
Key Responsibilities
Clinical & Resident Care Oversight
- Oversee and monitor resident care plans, ensuring services meet individual needs and comply with state regulations
- Recognize and respond to changes in condition; communicate effectively with nursing, families, and providers
- Support medication management processes in collaboration with licensed staff (QMAP experience strongly preferred)
- Ensure proper documentation, incident reporting, and follow-up
Leadership & Team Management
- Lead, coach, and hold accountable a team of CNAs/QMAPs and caregivers
- Hire, onboard, and train new staff with a focus on dementia care best practices
- Provide ongoing coaching, performance management, and corrective action when needed
- Foster a positive, team-oriented culture focused on accountability and resident-centered care
Operations & Scheduling
- Create and manage staff schedules to ensure appropriate coverage and budget adherence
- Approve timecards and manage labor efficiently
- Participate in on-call rotation to support staffing and resident needs
Regulatory Compliance & Quality Assurance
- Ensure compliance with all state regulations, company policies, and memory care standards
- Prepare for and participate in surveys and audits
- Maintain a safe, structured, and therapeutic environment for residents with dementia
Family & Communication
- Serve as a primary point of contact for families regarding care concerns and updates
- Support move-ins, assessments, and care transitions
- Address concerns professionally and proactively
Qualifications (What We Are Really Looking For)
- Required: Prior experience in Memory Care, Dementia Care, or Assisted Living leadership
- Required: Experience supervising or leading a caregiving team
- Preferred: QMAP certification (or ability to obtain quickly)
- Strong understanding of dementia behaviors and interventions
- Ability to manage conflict, hold staff accountable, and lead under pressure
- Excellent communication and organizational skills
- Compassionate, patient, and highly dependable
